Click on the link above to download and print the Patient Information Packet.  Once the entire form is complete, please fax to 586-443-5403 to allow the office time to input all information in the system prior to your appointment or bring the completed paperwork to your next office visit (this may cause a wait time to allow information to be put into the computer).

Patients MUST present photo ID and insurance card at the time of your office visit.  Failure to do so may result in rescheduling of your appointment.

NOTE: THE OFFICE ACCEPTS ALL CREDIT CARDS INCLUDING GOOGLE AND APPLE PAY.  WE DO NOT ACCEPT AMERICAN EXPRESS.
